Download FormMail.pl from www.scriptarchive.com then unzip and check out the readme file.
Formmail.pl is ideal and easy to implement with Unix based service providers. If your provider uses a windows server, contact the host for specific information on form to email processing. Formmail.pl is also available as a windows version, but only works if the service provider uses any of the following mail programs: "ASPMail", "CDONTS" or "JMail".
If you don't have an independent FTP program such as WS FTP Pro. I suggest you also download this from www.ipswitch.com
If you don't have $39 to spare, you can download WS FTP LE (still very good) from www.ipswitch.com/downloads/index.html by scrolling to the bottom.
