Finishing up! Place a form button below the table but inside the form area and name it Submit or whatever! Then preview in browser and test your form, fill it in and hit submit, then check your e-mail!
Warning: A form page or layout region must not be published fixed, or the form will not work in Netscape browsers. However, if using a layout region as seen below, you can set the html output of this region to dynamic while the rest of the site is published fixed.
